This report is a result of an onsite complaint investigation conducted on February 2, 2026 by staff from the Bureau of Program Licensure. Based on the findings of the complaint investigation, Fellowship House - Olyphant, O9716701 was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during this investigation.
