Plan of Correction The Notice of Treatment Termination letter that is sent to clients informing them of their discharge from services was revised by the Facility Director on 8/2/13 to include a statement informing clients of their right to request reconsideration of discharge. The revision also includes the contact person and contact information.
The Facility Director also informed all clinical staff and support staff of the revised form, distributed copies, and instructed to discard any copies of the previous letter and replace them with the revised letter, effective 8/2/13.
This change is in effect for all locations.
Clients are also informed of their right to request reconsideration of discharge in the Clients Rights they review and sign at intake.
