INITIAL COMMENTS
 
This report is a result of an off-site complaint investigation conducted on March 25, 2025 by staff from the Bureau of Program Licensure. Based on the findings of the off-site complaint investigation, YBHQ6701 Herkey House was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the recovery house. Therefore, no plan of correction is required.
