The following deficiencies were identified during this complaint investigation:

Plan of Correction

705.6 (7)  LICENSURE Bathrooms.

705.6. Bathrooms. The residential facility shall: (7) Maintain each bathroom in a functional, clean and sanitary manner at all times.
Observations
Based on a physical plant inspection conducted on April 22, 2016, at approximately 10:30 am to 12:00 pm, the facility failed to maintain sanitary conditions in a client bathroom located in the male wing, across from the group room and lounge area. This was evidenced by overflowing waste can, toilet paper lying on floor instead of in dispenser, wet wash cloth lying next to sink, and empty beverage bottle lying on counter top.

Plan of Correction
The Facility Director will be responsible to ensure that the male bathroom in question has its cleaning detail completed twice a day. One cleaning will occur at 9:30 AM and again at the scheduled cleaning detail of 4 PM. The facility director or designee will conduct a tour daily to ensure the completion of the cleaning detail.

The Facility Director will inspect all bathroom hygiene equipment to ensure they are operational and replace all broken equipment discovered during inspection.
