INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted on May 20, 2008 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Gaudenzia Focus House was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on June 24, 2008.

Plan of Correction

705.2 (1)  LICENSURE Building exterior and grounds.

705.2. Building exterior and grounds. The residential facility shall: (1) Maintain all structures, fences and playground equipment, when applicable, on the grounds of the facility so as to be free from any danger to health and safety.
Observations
Based on an interview with the acting facility director, the electrical service has been inoperative since May 19, 2008. Electrical service to the dining room, kitchen area and first floor/lower level were not operational during the inspection. The facility director failed to maintain the residential facility in a manner that would ensure the health and safety of the residents.

Plan of Correction
On May 21, 2008 Focus House contracted a certified electrical company to install new electrical panels. This work was monitored by Gaudenzia, Inc. Operations Department to ensure specifications required.

705.6 (7)  LICENSURE Bathrooms.

705.6. Bathrooms. The residential facility shall: (7) Maintain each bathroom in a functional, clean and sanitary manner at all times.
Observations
Based on an inspection of the residential facility, the facility failed to maintain clean bathrooms. Resident bathroom showers located on the first floor, third floor and fourth floor were not clean and well maintained. Shower stalls on all three levels had mold and soiled or stained shower bases.

Plan of Correction
Focus House line staff will monitor all bathrooms and shower stalls, clean weekly and report to Program Supervisor any concerns.

705.10 (a) (1) (iii)  LICENSURE Fire safety.

705.10. Fire safety. (a) Exits. (1) The residential facility shall: (iii) Maintain each ramp, interior stairway and outside steps exceeding two steps with a well-secured handrail and maintain each porch that has over an 18-inch drop with a well-secured railing.
Observations
Based on an inspection of the residential facility, the fire escape steps leading to the roof top did not have a railing. The residential facility failed to install and maintain a handrail on steps more than 18 inches in height.

Plan of Correction
Program Supervisor submitted work order to Gaudenzia Maintenance Department to install handrail which is scheduled for completion by July 5, 2008.
