INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted on November 13, 2008 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Mid-Atlantic Rehabilitation Services - A.T.P. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on December 10, 2008.

Plan of Correction

704.11(c)(2)  LICENSURE CPR CERTIFICATION

704.11. Staff development program. (c) General training requirements. (2) CPR certification and first aid training shall be provided to a sufficient number of staff persons, so that at least one person trained in these skills is onsite during the project's hours of operation.
Observations
Based on a review of three training records and an interview with the facility director on November 13, 2008 the facility failed to provide CPR and first aid training to a sufficient number of staff.

The findings include:

Two of three training records reviewed on November 13, 2008 did not document CPR or first aid training. Training record # 1 documented a first aid expiration date of November 2007 and training record #2 had a CPR expiration date of July 2008. Both staff are required to have CPR and First Aid training in order to provide for a sufficient number of staff to cover the hours of operation.

Plan of Correction
Facility Director will schedule annual first aid training for staff and staff will attend cpr training to ensure a sufficient number of staff have current certification. The facility director will audit personnell records to ensure a sufficient number of staff have first aid and cpr training to cover hours of operation. This will be scheduled by December 18, 2008 and will be conducted prior to January 18, 2009.
