Observations
Based on a physical plant inspection conducted during a complaint investigation on December 4, 2024, the facility failed to maintain each bathroom in a functional, clean and sanitary manner at all times.
The bathroom shower located in client bedroom 118 was observed to have a black mold-like substance in the right corner area of the shower walls.
The vinyl-like material shower covering was missing from the front of the shower step in the same bathroom.
This was discussed with facility staff at the time of the investigation.

Plan of Correction
In response to the bathroom in room 118; Maintenance remedied this by epoxying the shower basin on 12/17/2024. Ongoing inspection of the bathrooms will be a part of monthly physical plant walk-throughs facilitated by maintenance. Additionally, housekeeping will check for "black mold-like" substances in the showers during daily cleaning and remove if observed. |
