Based on a review of resident records, the recovery house failed to have a complete resident record.
These findings were reviewed with the recovery house staff during the licensing process.

Westminster has a complete resident records on file, during our re inspection for licensing there was one file missing from VH3B6701 files. As of 6/30/2024 Westminster will continue to keep a complete and accurate record of all Westminster records.
