This report is a result of an off-site complaint investigation conducted on July 17, 2026 through July 24, 2026 by staff from the Bureau of Program Licensure. Based on the findings of the off-site complaint investigation, F58Q6701 Herkey Houses LLC was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the recovery house. Therefore, no plan of correction is required.
