These findings were reviewed with facility staff during the licensing process.

Plan of Correction
The Outpatient supervisor will review all new client files, on a weekly basis, to ensure that all Release of Information forms are completed for the appropriate funding source, and signed by the client, prior to billing for any services. This was reviewed with all staff, including clerical staff, who oversees the billing of services. This was implemented on 10/07/2021.
