INITIAL COMMENTS

This report is a result of an on-site provisional license follow-up inspection conducted on April 30, 2026 by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the on-site inspection, Auberle was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

Plan of Correction

709.28 (a) (1)  LICENSURE Confidentiality

§ 709.28. Confidentiality. (a) A written procedure shall be developed by the project director which shall comply with 4 Pa. Code § 255.5 (relating to projects and coordinating bodies: disclosure of client-oriented information). The procedure must include, but not be limited to: (1) Confidentiality of client identity and records. Procedures must include a description of how the project plans to address security and release of electronic and paper records and identification of the person responsible for maintenance of client records.
Observations
Based on a physical plant inspection, the facility failed to maintain confidentiality of client records. The facility's procedure states that all hard copy client records are stored within locked storage containers and electronic health records are only accessible with password-protected log-in credentials. Medication records were observed to be in multiple, unsecured piles throughout the medication room.

This finding was reviewed with facility staff during the licensing process.

Plan of Correction
Upon identification of this finding, all medication records located in the medication room were immediately collected, organized, and secured to protect client confidentiality. To address the deficiency and prevent recurrence, the Family Healing Center purchased and installed a locking filing cabinet within the medication room for the secure storage of all medication administration records and related client documentation. Staff were re-educated on confidentiality requirements and the expectation that all medication records be returned to the locked filing cabinet immediately after use. Supervisory staff will conduct routine audits of the medication room to ensure records remain properly secured and that confidentiality standards are consistently maintained. Compliance with these procedures will be monitored through the facility's ongoing quality assurance process.

709.32 (c) (2)  LICENSURE Medication control

§ 709.32. Medication control. (c) The project shall have and implement a written policy and procedures regarding all medications used by clients which shall include, but not be limited to: (2) Drug storage areas including, but not limited to, the secure storage of controlled substances and other abusable drugs in accordance with State and Federal regulations and program requirements.
Observations
Based on a physical plant inspection, the facility failed to maintain their procedures regarding all medications used by clients, including drug storage areas. The facility's policy states that keys and access codes are limited to authorized staff and must remain under staff control at all times. During inspection of the medication room, the medication cart keys were observed, unattended, on the medication cart. There were no staff present in the medication room at the time of the inspection.

This finding was reviewed with facility staff during the licensing process.

Plan of Correction
Upon identification of this finding, the medication cart keys were immediately secured, and staff were re-educated on the facility's medication security policy requiring that medication cart keys remain under the direct control of authorized staff at all times when in use. To strengthen compliance, the Family Healing Center implemented a designated key control process whereby medication cart keys are either maintained on the authorized staff member's person or secured within a locked safe when not actively in use. Access to the safe is limited to authorized personnel through a secure access code. Staff have been trained on the updated procedure and expectations regarding medication security. Supervisory staff will conduct routine monitoring of medication storage areas and key management practices to ensure ongoing compliance with medication security requirements and prevent recurrence of this issue.

709.32 (c) (4) (i) - (ii)  LICENSURE Medication control

§ 709.32. Medication control. (4) Methods for control and accountability of drugs, including, but not limited to: (i) Who is authorized to remove drug. (ii) The program ' s system for recording drugs, which includes the name of the drug, the dosage, the staff person, the time and the date.

This is a repeat citation from the January 21, 2026 annual licensing inspection.

This was finding was reviewed with facility staff during the licensing process.

Plan of Correction
Upon identification of this finding, the facility conducted a comprehensive review of medication administration records and medication management practices to identify gaps in documentation and oversight. While medications may have been administered as prescribed, documentation was not consistently completed in accordance with facility policy, and there was insufficient documentation regarding medication refill efforts and physician communication when medications became unavailable. To address this deficiency, the Family Healing Center implemented a revised medication administration and documentation process that requires staff to document medication administration immediately following administration and to document all medication refill requests, pharmacy communications, physician notifications, and medication availability issues. In addition, a Medication Refill Tracking Log has been implemented to monitor medication inventory and proactively address refill needs before medications are depleted. Supervisory staff will conduct weekly MAR audits and monthly quality assurance reviews to verify medication administration documentation, identify discrepancies, and provide corrective coaching as needed. Medication administration staff have been re-educated on documentation requirements, medication accountability standards, and procedures for addressing unavailable medications. Ongoing monitoring will continue through the facility's quality assurance program to ensure sustained compliance and prevent recurrence of this finding.
