INITIAL COMMENTS

This report is a result of an on-site inspection conducted for the approval to use a narcotic agent, specifically methadone, in the treatment of narcotic addiction. This inspection was conducted on April 29-30, 2014 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Parkside Recovery was found not to be in compliance with the applicable chapters of 4 PA Code and 28 PA Code which pertain to the facility.

Plan of Correction

715.21(2)  LICENSURE Patient termination

A narcotic treatment program shall develop and implement policies and procedures regarding involuntary terminations. Involuntary terminations shall be initiated only when all other efforts to retain the patient in the program have failed. (2) A patient terminated involuntarily, except a patient who commits or threatens to commit acts of physical violence, shall be afforded the opportunity to receive detoxification of at least 7 days. The detoxification may take place at the facility or the patient may be referred to another narcotic treatment program or hospital licensed and approved by the Department for detoxification.
Observations
Based on the review of patient records, the facility failed to afford the opportunity to the patient to receive a detoxification of at least 7 days in one of two patient records reviewed.

The findings include:

Nine patient records were reviewed April 29-30, 2014. Two patient records required documentation the patient was afforded at least a seven day detoxification. One patient was not afforded the 7 day detoxification period.

Patient # 7 was admitted October 9, 2012. The patient had medical problems off and on which required hospitalization. The patient's most recent hospitalization ended and the patient was "verbally" reinstated February 23, 2014. The patient was to see the physician February 24, 2014. Prior to seeing the physician on February 24, 2014, the patient was observed dealing illicit substances. The patient was not seen for reinstatement by the physician, but was instead given a notice of immediate termination without the benefit of at least a seven day detoxification.

Plan of Correction
It is NHS policy that any client returning from a hospitalization stay be re-instated in the program before any other decision regarding the client's continued stay in treatment be made. Patient # 7 should have been re-instated in the program before the decision to give a letter of termination was made. If then a letter of termination was given the patient should then have been afforded a 7 day detox. The policy on re-instatement and the policy on letters of termination were reviewed with the Program Director, Clinical Supervisor, Physician and Nursing Supervisor to assure that both policies are followed in all cases. The Program Director will be responsible to assure compliance.
