The administrative Assistance will track and monitor follow up calls including sending letters when contact is unable to be made, this will allow for more oversight and tracking of follow up calls and allow for documentation of follow up calls to be made in the patient record and not to have external documentation.
Monthly reviews will occur with managers and Admin assistants to monitor that follow up calls are made and documented appropriately
Mangagers and Directors of the programs will be responsible for oversight and accountability.
Aftercare plans are completed as a group and reviewed with each client's individual therapist to be retained in the client record. Aftercare plans or refusal to complete aftercare plans will be documented in the client record in the appropriate group note as well as the aftercare plan itself. |
