This report is a result of an on-site complaint investigation conducted on August 11, 2016 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site complaint investigation, North Philadelphia Health System Dd/fir was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.

709.14. Restriction on license.
(b) The licensee, using Department forms, shall notify the Department within 90 days of the occurrence of any of the following conditions:
(4) Change in activity/discontinuance of an activity.

Observations
Based on a complaint investigation and interviews with facility staff, the facility was providing intake, evaluation and referral activities that they were not licensed to conduct.
The finding was reviewed with facility staff during the complaint investigation.

Plan of Correction
The facility has stopped the provision of assessment and referral services as of October 8, 2016 and will not provide these services until they have been approved by DDAP.
The Sr. Vice President has submit a request to add the activity of intake, evaluation and referral activities to its outpatient license # 800244. |
