INITIAL COMMENTS
 
This report is a result of a complaint investigation conducted by telephone on July 8, 2011 by staff from the Division of Drug and Alcohol Program Licensure. Based on the the findings of the complaint investigation, Gaudenzia, Inc Fountain Springs was not in violation of the applicable chapters of 28 PA Code which pertain to the facility. No plan of correction is required.
