Plan of Correction
|
Observations The facility failed to develop an appropriate preliminary treatment and rehabilitation plan for client #1.
Client #1 completed a Drug and Alcohol assessment at the facility on March 17, 2020. On May 11, 2020 a letter was sent to client #1 to inform him he was being discharged for not attending treatment. On June 8, 2020 client #1 was provided the document called " Drug and Alcohol Initial Documentation " from the facility; there is no substance use disorder diagnosis on this document and the " Treatment Program " to which the client was admitted was identified as " MH-OP " (mental health outpatient). There is a ' Preliminary Treatment Plan ' section on page 2 of this form which is completed but contains substance use disorder treatment goals and action steps.

Plan of Correction On July 22, 2020, the Clinical Supervisor met with and provided re-education to the counselor who completed the assessment on May 11, 2020. The document was amended to include a substance use disorder. The counselor has elected to no longer accept referrals for drug and alcohol assessments due to the heavy clinical documentation burdens and external oversight required for SUD treatment.
Through group and individual staff meetings, the Clinical Supervisor will continue to provide ongoing supervision and feedback to staff to ensure that assessments are completed thoroughly and accurately. The Quality Improvement Department will complete random chart audits to ensure that appropriate preliminary treatment and rehabilitation plans are developed. |
