Observations
The facility failed to ensure the front door emergency exit was unobstructed based on a physical plant inspection conducted on December 13, 2018 at approximately 9:15 AM.
The facility has a glass front door that has a double-keyed deadbolt lock. This lock would prevent the exit from being operable at all times.
The findings were reviewed with facility staff during the licensing process.

Plan of Correction
On December 14, 2018, the Community Counseling Center Organizational Development Director arranged for new turn lock to be installed by the maintenance staff. Maintenance staff installed the new turn lock on December 19, 2018. The double-keyed deadbolt lock on the glass front door was replaced with a new lock that is hand latched on the inside and keyed on the outside. The replacement lock is a Schlage Light Commercial Style B60NCSV626 Model-6324169121.
The facility director will ensure all emergency exits are unobstructed by completing a physical check of the building exits monthly.
