This report is a result of a telephone complaint investigation conducted on November 9, 2022 and November 10, 2022 by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the complaint investigation, Gaudenzia, Inc. Fountain Springs was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
