Observations
Based on a review of facility policy, the facility failed to document a written patient follow-up policy that addressed contacting the emergency contact when a patient leaves AMA.
These findings were reviewed with facility staff during the inspection process.

Plan of Correction
Policy and Procedure for Follow-up was updated to include contacting the emergency contact when a patient leaves AMA within 12 hours of the patient signing out of the program. The counselor or designee will document these correspondences in the electronic medical record, and if emergency contact doesn't answer a message will be left on the voicemail. All staff will be educated on the change in policy and will sign stating that they have participated in the education and/or read the information regarding the change and agree to follow the policy. The education of all staff working with detox patients will be completed no later than 8/31/22. |
