Observations
Based on a physical plant inspection, the facility failed to ventilate the first floor men's bathroom by exhaust fan or window.
The findings include:
A physical plant inspection was conducted with the Project/Facility Director on March 14, 2013 at approximately 11:45 AM. The first floor men's restroom, near the reception area, was observed to not have an operable exhaust fan or window.
The Project/Facility Director was interviewed on March 14, 2013 and confirmed the findings.

Plan of Correction
Exhaulst fan has already been installed and is fully functioning in the men's room, 1st floor.
Project Director shall:
1.Secure licensed electrician to purchase replacement exhalst fan
by 03-27-2013
2.Secure licensed elctrician to install exhalst fan by 03-27-2013
3.Confirm operation of exhalst fan operating by 03-27-2013
4.Continue to check 1st floor men's bathroom and all other bathrooms in facility at least monthly to insure units are functioning.
