Observations
Based on observation during a physical plant tour with the Facility Director, the facility failed to insure that individual bars of soap in shared resident bathrooms were in labeled containers in four of the resident bathrooms.
Findings:
A tour of the facility physical plant was conducted on November 17, 2009 between 10:00 a.m. and 10:35 a.m. with the Facility Director present. During the tour, all eight resident rooms and adjoining bathrooms were observed. In rooms 160, 161, 164 and 165 where two clients shared a bathroom, soap bars were observed in the shower/tub areas and/or sink. The bars of soap appeared to have been used, were observed in the shower, tub or sink soap holders and were not contained in individual marked soap containers.

Plan of Correction
Individual soap containers are being ordered for those residents who wish to use bar soap. The soap dispensers will also be available for individual use. |
