INITIAL COMMENTS

This report is a result of an unusual incident report investigation conducted on April 3, 2015 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the investigation, Pa Care. Llc D/b/a State College Medical was found to be in compliance with all applicable regulations. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
