This report is a result of a complaint investigation conducted by telephone on November 12, 2014 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation, the allegations against Pyramid Healthcare, Inc. were unable to be substantiated based on the facility policy. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
