INITIAL COMMENTS
This report is a result of an onsite physical plant inspection conducted on September 12, 2017 by staff from the Division of Program Licensure for the licensure renewal. Based on the findings of the inspection, North Philadelphia Health System Goldman Clinic was found to be out of compliance 28 Pa. Code chapter 705.21 Physical Plant Standards. Because this was just a physical plant inspection not all regulations were looked at during this inspection. The facility has the following deficiencies:	Plan of Correction
705.23 (3) LICENSURE Counseling or activity areas and office space 705.23. Counseling or activity areas and office space. The nonresidential facility shall: (3) Ensure privacy so that counseling sessions cannot be seen or heard outside the counseling room. Counseling room walls shall extend from the floor to the ceiling.
Observations Based on observation on September 12, 2017, the facility failed to ensure that counseling sessions could not be heard outside of the counseling areas. The findings include the following: Counseling sessions were able to be heard outside of the Group Room and counseling office on the fourth floor. Counseling sessions were able to be heard outside of counseling office 232 on the second floor.	Plan of Correction NPHS will purchase white noise machines on or before October 28,2017 to be place outside/inside of group and counseling offices to eliminate the ability for sessions to be heard outside of the rooms. Clinical supervisors will monitor on a monthly basis the areas to ensure that counseling sessions can not be heard outside of the rooms.
