This report is a result of an on-site complaint investigation conducted on April 28, 2025, by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, 79 Blueridge, LLC was found to be in non-compliance with the applicable chapters of 28 PA Code, Chapter 717, which pertains to the recovery house.

§ 717.29. Physical plant standards.
(d) Sleeping accommodations.
(1) Each drug and alcohol recovery house bedroom must have the following:
(i) A bed in good repair with a solid foundation and fire-retardant mattress for each resident.

Observations
Based on an inspection of the physical plant and resident interviews it was determined the recovery house failed to provide a bed in good repair with a solid foundation and fire-retardant mattress for one resident.

Plan of Correction
As of 4/30/2025 the House Manager of 79 Blue Ridge Dr LLC is responsible for keeping an up to date and accurate census of all houses. As of 4/30/2025 House manager is also keeping an up-to-date white board of all houses and current residents so overbooking will not occur. In the event of an emergency House manager will notify DDAP by completing a Recovery House Exception Request Form immediately. |
