Plan of Correction
The plan of corrections were not followed up on from previous audits. This has been addressed with the CEO, Clinical Director and Facility Director. This was also addressed in clinical meeting on 8/19/24. There will be consequences moving forward if clinical staff is not able to comply with having their charts stay in compliance.

Clinical Director will be going over trainings during staff meetings once a month on issues that occurred during this and previous corrective action plans to ensure this does not reoccur. Clinical Director will also be auditing charts on a weekly basis and clinical staff will be getting that once a week on Mondays.
