The findings include:

The facility's staff schedule for July 16, 2012 through August 7, 2012 and CPR/first aid cards were reviewed on August 7, 2012. The facility provides residential treatment and rehabilitation services 24 hours per day.

The facility failed to ensure that at least one person trained in CPR and first aid was onsite during the hours of 11 PM and 7 AM on the following dates: July 27 - 29, July 31 and August 3 - 5, 2012.

The facility director was interviewed throughout the licensing inspection and confirmed the findings.

Plan of Correction
The Facility had a staff member that was qualified to train staff in CPR and First Aid which was no longer able to continue to train staff due to various outside commitments. The Facility attempted to find another place that offered CPR and First Aid. It was discovered that local residential treatment facilities were using online trainings for CPR and First Aid. At that time all individuals that were required to have CPR and First Aid took the course online and received certificates. The agency became aware that this course did not meet the CPR and First Aid Standards. The Two Behavioral Tech's were scheduled for CPR and First Aid on 8/23/12. The CEO will ensure that the CPR and First Aid course meets that standards. The Fiscal/Administrative Assistant will ensure that the individuals are trained in CPR and First Aid when working alone. The Facility will be in full compliance by 8/24/12.
