This report is a result of a licensure renewal inspection consisting of an onsite physical plant inspection completed on September 1, 2022, and offsite client record review conducted on September 2, 2022, by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the inspections, Pyramid Healthcare Inc, Erie Outpatient was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. No deficiencies were identified during this inspection and no plan of correction is required.
