Observations
The facility failed to document an informed and voluntary consent from the client prior to the disclosure of information contained in the client record in one of fourteen client records reviewed on September 23, 2019.
Client # 8 was admitted May 7, 2019. The facility disclosed the name and request for funding authorization for client # 8 to the funding entity by fax on May 13, June 4 and July 18, 2019.
The findings were reviewed with facility staff during the licensing process.

Plan of Correction
The scheduler, counselor and Manager of OP Services will continue to complete chart audits monthly, focusing on the completion of consents.
Each month the scheduler will send the Manager of OP Services a list of 10 client charts they specifically cross referenced consents to correspondence that was sent by the counselor.
Any counselor who is identified as a consistent staff member that is not completing consents will be reeducated on the importance of consents and rules of confidentiality. |
