Plan of Correction
The facility failed to comply with 709.28(d) related to confidentiality of a client consent to be offered to the client and a copy maintained in the client record. Based on these findings the Executive Director submitted to the electronic medical record system to include the information to reflect the ability to accept or decline a copy of the consent. The electronic medical record system updated the consent on 12/7/2023. The Executive Director retrained the clinical team on 12/8/2023 to offer the client a copy of all releases and ensure the client accepts or declines all consent forms. The Executive Director and Chief Operating Officer will conduct monthly chart audits to ensure all consent forms are completed correctly.
