INITIAL COMMENTS
This report is a result of an on-site complaint investigation conducted on September 24, 2024 by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Alpine Springs Rehabilitation & Recovery Center LLC was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.	Plan of Correction
705.6 (5) LICENSURE Bathrooms. 705.6. Bathrooms. The residential facility shall: (5) Ventilate toilet and wash rooms by exhaust fan or window.
Observations Based on a physical plant inspection, the facility failed to ventilate toliet and wash rooms by an exhaust fan or window. The exhaust fan in the restroom off of the large common room was found to be inoperable at the time of the physical plant inspection. There is no window present in this restroom.	Plan of Correction There is a exhaust fan located in the restroom, it stopped working. A new fan was installed on 10/03/2024, This is now resolved and in complaince to ventilate wash rooms by an exhaust fan. This will be monitored each month by the facility manager during their monthly safety round checks. It was added to the form that they complete monthly, which is signed off by the manager and facility director.
