Observations
Ten client records were reviewed on December 22, 2016. Based on this review, the facility failed to review and update three client treatment and rehabilitation plans within 60 days.
Client #3 was admitted on August 1, 2016 and was still active an active client. The treatment and rehabilitation plan was completed on 8/9/16 and the update was not completed until 10/31/16
Client #9 was admitted on June 9, 2016 and was discharged December 21, 2016. The treatment and rehabilitation plan was completed on 9/1/16 and the update was not completed until 11/26/16.
Client #10 was admitted on July 14, 2016 and was still an active client. The treatment and rehabilitation plan was completed on 7/28/26 and the update was not completed until 10/5/16.
These findings were reviewed with facility staff during the licensing inspection.
