This report is a result of an on-site complaint investigation conducted on May 26, 2021 by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Samara House of CYWA was found to be in compliance with applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during the investigation and no plan of correction is required.
