Observations
During the complaint investigation conducted on October 1, 2021, the facility failed to ensure that heaters are mounted. There was heater located in the Facility Director's office that was not permanently mounted.
This was discussed with the Facility Director during the complaint investigation.

New Castle Comprehensive Treatment Centers corrective plan of action is as follows:
1.Remove the floor heater from New Castle Comprehensive Treatment Center 2710 W. State Street New Castle PA 16101
-Completed on 10/1/21
2.Purchase a wall heating unit to replaced the floor unit
- Completed on 10/2/21
Received delivery of the Infrared Heat Storm wall unit, Model HS-1000-WXG was delivered from Amazon.
- Completed 10/03/21
3.Wall Unit was installed on the Director's office wall
-Completed on 10/11/21.
4.Floor heating units will not be used at 2710 W. State Street New Castle PA 16101
