Observations
Based on a review of the client records, the facility failed to document treatment plan updates every 60 days in one of two records reviewed.
Client #2 was admitted to treatment on July 29, 2019 and discharged on November 16, 2021. A treatment plan update was documented on July 26, 2021. There were no other treatment plans documented.
The findings were reviewed with facility staff during the licensing process.

Plan of Correction
The Executive Director reviewed with the Clinical Director the necessity of counselors to complete treatment plan updates every 60 days for those in the outpatient program. The Clinical Director will review this with the clinical staff at the next staff meeting on 4/7/22. The Clinical Director and supervisory staff will monitor treatment plan due dates during supervision to ensure treatment plans are updated every 60 days, with corrective action plans developed if this fails to occur. A report will be created in the EHR to monitor these treatment plan due dates. |
