INITIAL COMMENTS

This report is a result of an on-site initial licensure inspection conducted on July 20, 2023 by staff from the Bureau of Program Licensure. Based on the findings of the on-site inspection Rios Recovery was found to not be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, deficiencies were identified during this inspection and a plan of correction is required.

Plan of Correction

§ 717.29(d)(1)(ix)  LICENSURE Physical plant standards - Sleep accomodation

§ 717.29. Physical plant standards. (d) Sleeping accommodations. (1) Each drug and alcohol recovery house bedroom must have the following: (ix) A window with a source of natural light.
Observations
Based on the physical plant inspection the recovery house failed to ensure that there was a window in the basement bedroom as a source of natural light.

Plan of Correction
Due to our small window not being big enough in our basement room and for it not being able to be easily accessed to, Rios Recovery has decided to close down our basement room until we can possibly figure out how to install a window in the room. The resident had already moved upstairs to a different room about 3 weeks ago and that room is closed off and used for any extra storage. I've already shared the photos with Antoinette. Date of correction was 08/01/2023

§ 717.30(b)(1)  LICENSURE Safety and emergency procedures - Exits

§ 717.30. Safety and emergency procedures. (b) Exits. The licensee shall: (1) Ensure that stairways, hallways and exits from rooms and from the drug and alcohol recovery house are unobstructed.
Observations
Based on the physical plant inspection the recovery house failed to ensure that stairways, hallways, and exits from rooms and from the drug and alcohol recovery house are unobstructed. Access to the exits was obstructed by debris and air conditioners.

Plan of Correction
We were informed that our basement exit needed to be cleaned up on the stairs and to add a railing. We have cleaned up all the debris and have easy access in and out of the exit. We also provided the railing on the stairs as well. Pictures of corrections were already sent to Antoinette. Date of correction was 8/16/2023
