The individual plans for the employees listed were updated in writing as of 12/30/21 to include relevant trainings during the remainder of the training year until such time as a review of the electronic system occurs to prevent any similar problem in the future. Moving forward, the HR Director (or designee) is responsible for ongoing monitoring for completion of training plans prior to filing in personnel records. Any variances identified will be brought to the attention of the relevant manager/director for remediation with their staff. Any findings will be discussed monthly in rehab dashboard meeting.
