This report is a result of an unusual incident report conducted on February 5, 2015 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the unusual incident with White Deer Run- Allenwood , the findings were unable to be substantiated. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
