INITIAL COMMENTS

This report is a result of an onsite follow-up inspection regarding the plans of correction for the May 29, 2009 complaint investigation. The follow-up inspection was conducted on July 29, 2009 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the onsite follow-up inspection, Gaudenzia Fountain Springs was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on August 14, 2009.

Plan of Correction

705.2 (2)  LICENSURE Building exterior and grounds.

705.2. Building exterior and grounds. The residential facility shall: (2) Keep the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well-being of residents, employees and visitors. The exterior of the building and the building grounds or yard shall be free of hazards.
Observations
Based on a plan of correction follow up inspection conducted on July 29, 2009, the facility had installed door handles on exit doors that could be locked, thus preventing egress from those marked exits.

The findings include:

A plan of correction follow up inspection was conducted on July 29, 2009 to determine if a safe environment existed for staff and clients during the ongoing construction. Marked exits from the third floor of the Fountain Springs program went through doors that had been equipped with door handles that could be locked, thus preventing egress from that exit. The doors were not locked at the time of the unannounced inspection. The locks have to be removed from those doors, or panic bars that would override the locks would have to be installed.

Plan of Correction
On July 29, 2009, the door locks were removed from the marked exit doors on the 2nd and 3rd floors. Passage sets will be installed on marked exit doors by 8/27/09. Program Director will ensure completion.
