This report is a result of an on-site complaint investigation conducted on August 9, 2023 by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Phoenix House was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.

§ 717.22(a)(1) LICENSURE Beginning of Residency
§ 717.22. Beginning of residency.
(a) The licensee shall develop and implement written policies and procedures for beginning of residency which include:
(1) Residency criteria.

Observations
Based on a review of the recovery house residency criteria, and category selected for the recovery house(male). The recovery house failed to implement policy by allowing a female to sleep over at the recovey house.
This information was discussed with the house manager during the investigation.
