INITIAL COMMENTS

This report is a result of a complaint investigation conducted on October 5, 2016 by staff from the Division of Accountability and Program Improvement. Based on the findings of the on-site, the allegations against Summit Medical Services were substantiated. Therefore, the facility was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

Plan of Correction

715.7(b)  LICENSURE Dispensing or Administering Staffing

(b) Dispensing time shall be prorated for patient census. There shall be sufficient dispensing staff to ensure that all patients are medicated within 15 minutes of arrival at the dispensing area.
Observations
Based on patient interviews conducted during the on-site visit on October 5, 2016 Summit Medical Services failed to ensure that all patients were medicated within 15 minutes of arrival at the dispensing area.

It was noted by the facility director that on 07/09/16-07/10/2016 dosing times exceeded 15 minutes.

Plan of Correction
On the dates indicated (7/9 and 7/10), patients were not medicated within the 15 minutes of arrival time. This occurred due to staff not being on site timely due to a family emergency.

Facility director - Ian Winter - conducted a staff meeting on 10/20/16 to address this issue. Addressed were measures put into place to ensure staff being on-site if another staff had an emergency situation. Call off protocol was reviewed as well as a staff back-up system in case of emergencies. Staff are to give at least a 1-hour notice if calling off for an emergency. Also during weekend (short-staffed days), staff will be on-hand 1/2 hour earlier and will notify on-call staff (clinical supervisor or director) if other staff are not present. That will give ample time for back-up or supervisory staff to get on-site timely, thus ensuring all patients are dosed within 15 minutes of arrival.

This will be carried our as of 10/20/16 by Facility director - Ian Winter
