(c) Clinical supervisors and lead counselors who have not functioned for 2 years as supervisors in the provision of clinical services shall complete a core curriculum in clinical supervision. Training not provided by the Department shall receive prior approval from the Department.
Observations
Based on a review of personnel records, the facility failed to ensure that clinical supervisors who have not functioned for 2 years as a supervisor in the provision of clinical services completed a Department-approved core curriculum in clinical supervision in one of one applicable personnel record reviewed. Employee #3 was hired as a clinical supervisor on September 7, 2021 and did not have at least 2 years of clinical supervision experience prior to being hired at the facility. There was no documentation of the clinical supervision core curriculum training on file at the time of the inspection. This finding was reviewed with facility staff during the licensing process.

Plan of Correction
The facility director will secure the necessary training to fulfill the less than 2 year supervisor experience requirement for the clinical supervisor. The facility director will send weekly emails to DDAP TMS and the other approved sites to check for availability until securing a spot. We are hoping to secure a spot by 8/31/2022.

In the future when a clinical supervisor is hired with less than 2 years of supervisory experience the facility director will send weekly emails to all state approved sites for supervisory training until securing a spot with in the first six months of employment of that supervisor.

The project director will follow up with facility director monthly during supervision meetings to ensure we are doing everything in our power to secure a training spot for any new hires and our current clinical supervisor.
