Observations
Based on a review of client records, the facility failed to notify the client, in writing, of a decision to involuntarily terminate the client's treatment at the project in one of two applicable client record.
Client #12 was admitted to the outpatient level of care on May 20, 2024 and discharged on June 20, 2024.
This finding was reviewed with the facility staff during the licensing process.

Plan of Correction
It shall be the responsibility of the Program Manager to train program staff on the proper completion of notifying patient in writing when involuntarily terminated from the program by August 20, 2024.
The Program Manager shall be responsible for verifying accurate completion of the releases by completing random chart supervision in 10% of the charts monthly. |
