INITIAL COMMENTS

This report is a result of an on-site inspection conducted for the approval to use a narcotic agent, specifically buprenorphine, in the treatment of narcotic addiction. This inspection was conducted on December 10, 2010 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Gaudenzia, Inc. Fountain Springs, was found not to be in compliance with the applicable chapters of 4 PA Code and 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on January 5, 2010.

Plan of Correction

715.6(a)  LICENSURE Physician staffing

(a) A narcotic treatment program shall designate a medical director to assume responsibility for administering all medical services performed by the narcotic treatment program
Observations
Based on an interview with the facility director and division director, the facility failed to designate a medical director to assume the responsibility for administering all medical services performed by the narcotic treatment program..

The findings include:

On December 10, 2010 the facility director and division director confirmed that there had not been a medical director at the facility since May 2010 when the physician resigned. The facility director and division director stated they have been trying to fill the position but have not been successful.

Plan of Correction
A physician with experience/certification in the treatment of narcotic addiction with suboxone will be hired as the Medical Director for the Detox Unit. Efforts to fill the position will include: advertising in newspapers; contacting local hospitals and medical centers; contacting doctor offices; and following up on referrals on community contacts. Steps to find a physician were started several months ago, however, filling this position has been difficult. There is a shortage of physicians in this rural area and those we have contacted have indicated they are unable to take on additional responsibilities. Therefore, the search will be expanded to outlying areas and we will re-contact those with whom we spoke previously. The Program Director and Division Director will take an active role in and oversee this process.
