This report is a result of an off-site initial licensure inspection of the pre-survey material submitted for the addition of a Partial Hospitalization Treatment Activity to the existing Outpatient Treatment activity. The review was completed on March 13, 2013 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the pre-survey process, Jade Wellness Center LLC, was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the Partial Hospitalization Activity.
Since this was the initial review of the Partial Hospitalization Activity, not all regulations were reviewed. During future inspections, all regulations will be reviewed for compliance.
