INITIAL COMMENTS
This report is a result of an on-site licensure renewal inspection conducted on September 25, 2006 by staff from the Division of Drug and Alcohol Program Licensure. The following deficiencies were identified during this inspection and a plan of correction is due on October 24, 2006.	Plan of Correction
709.28(c) LICENSURE Confidentiality 709.28. Confidentiality. (c) The project shall obtain an informed and voluntary consent from the client for the disclosure of information contained in the client record. The consent shall be in writing and include, but not be limited to:
Observations The consent to release information form for criminal justice entities stated that the consent could not be revoked until certain conditions were met in three of eight records reviewed, #1, 3 and 5. The consent to release information to third party payers exceeded that allowed by 4 Pa. Code 255.5 in two of eight records reviewed, #6 and 8	Plan of Correction An inservice was conducted during staff meeting on 10-3-06 on received cites. PA code 255.5 was reviewed. New release forms were handed out, administrative coordinator destroyed all old non-compliant releases. New releases in files # 1 and # 3 have since been added.
