This report is a result of an initial on-site physical plant inspection preceding a facility relocation. The inspection was conducted on May 9, 2013 by staff from the Program Licensure Division. Based on the findings of the physical plant inspection, Pyramid Healthcare Inc. - Transitions 2 was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
Since this was a physical plant inspection preceding a facility relocation, not all regulations were reviewed. During future inspections, all regulations will be reviewed for compliance.
