Observations
Based on a tour of the physical plant, the facility failed to light interior exits and stairs at all times.
The findings include:
A tour of the facility was conducted on June 16, 2010 at 10:35 AM. During the tour it was found the light bulb was burnt out in the stairwell from the third floor.
Additionally, the light near the basement exit was out and the light switch was not working.

Plan of Correction
All interior exits and stairs will be lit at all times and all light switches will be in working order. Lighting will be checked daily by staff on duty and repairs, if needed, will be made in a timely manner. Signage directing that lights must remain on at all times will be posted. Program Director will ensure all staff is aware of regulation via written correspondence and staff meeting. Program Director will ensure compliance. |
