Observations
Based on a review of personnel records, the facility failed to have the counselor complete at least 25 clock hours of annual training in one of two applicable records.

Staff #4 was hired as counselor on June 27, 2022, and still employed at the time of inspection. Staff #4 documented 18 hours of training for the January 1, 2025- December 31, 2025 training year.

This finding was reviewed with facility staff during the licensing process.

Plan of Correction
All Counselors will receive at least 25 hours of training annually; January-December. Beginning 8/3/26, supervisors will monitor amount of training hours completed each month in our training portal by running a report the 1st week of each month and assigning trainings as appropriate to ensure compliance of required training hours before the end of the year/December. Required training expectations will be reviewed with all staff in an upcoming meeting on 8/20/26.
