The findings include:An onsite complaint investigation was completed on November 10, 2014. One client record was reviewed. As a result of the complaint investigation, it was found that the facility's procedures for the management of treatment/rehabilitation services for clients did not include notifying the Department of an incident requiring the services of a law enforcement agency nor the safety of the clients. The facility did not notify the Department of an incident where, as the result of a client leaving the facility grounds and allegedly committing a criminal act, police had come to the facility to interview individuals at the residence. The facility was not aware the client had left the facility until the police arrived at the facility. These findings were reviewed with facility staff during the complaint investigation.
